How do I make an appointment?

Simply contact us by phone, e-mail, or stop in personally. We will have you fill out an information and medical history form, a federal HIPPA privacy form, and insurance authorization, if applicable. These forms can also be mailed to you in advance for you to fill out at your convenience.

 

Can I see the dentist or hygienist of my choice?

Absolutely! If you have a preference, please let us know when you make an appointment and we will schedule you accordingly.

 

Do you process my insurance?

Although we do not participate as a provider directly for Delta Dental, Met-Life, or Aetna, we will process and submit your claims for you. We will generally process your claims electronically the same day that you are seen.

 

When is payment due?

Payment is due at the time of service unless other arrangements have been made. We do accept Mastercard, Visa, and Discover cards, as well as cash or check. A service charge begins to apply on any balances beyond sixty days. If you would like to arrange a payment plan, you can set up an appointment with Pat Thayer, our financial coordinator.
